I expect that my child will attend the following school session with Mt. Lebanon School District

Hybrid Support Program Schedule

Please check the days per week that your child will need care for each of the sessions below. 
Minimum enrollment for grades 1-5 is 3 days per week/per session
Due to the hybrid learning schedule, the minimum enrollment for Kindergarten Care is 2 days per week. 

AM CARE (7:00 - 8:30 AM)
MLEDP HALF DAY CARE SESSION 1 (8:30 AM - 12:45 PM). *This means that your child will attend school from 12:45 - 3:30 PM (Last names ending in M-Z and children assigned to the Afternoon Kindergarten Class & their siblings)
MLEDP HALF DAY CARE SESSION 2 (11:15 AM - 3:30 PM). *This means that your child will attend school from 8:30 - 11:15 AM (last names ending in A-L and children assigned to the Morning Kindergarten Class & their siblings)
PM CARE (3:30 - 5:30 PM)
KINDERGARTEN ONLY: ALL DAY CARE (8:30 AM - 3:30 PM)

Contract for Hybrid Support Programming:

In addition to the MLEDP before and after care contract, the following will apply to hybrid Support programming. 

Please check each box to indicate your understanding and agreement with each section:
I understand that MLEDP will support the school district Hybrid Learning Model by assisting and monitoring children in the remote school environment. Staff will assist children to log onto assignments, assist in understanding and reading instructions, help children contact their teacher for questions or to request office hours. Staff will not serve in the capacity of tutor or subject matter specialists. Staff will ensure that children attend their remote asynchronous and synchronous subjects but cannot assure that all work is complete and accurate. Parents will be given a progress report at the end of each day. MLEDP will operate from 7:00 AM - 5:30 PM. Monday - Friday.
I give my permission for my child to use hand sanitizer provided by MLEDP
I have read the Health and Safety Plan located on the MLEDP website (Linked Below). I agree to abide by all requirements of the plan. I understand exposure to COVID-19 can have severe health consequences up to and including death and that every effort has been made to mitigate exposure to COVID 19. By signing this contract I ASSUME ALL SUCH RISKS and RELEASE, HOLD HARMLESS AND INDEMNIFY MLEDP, the Mt. Lebanon School District should my child contract COVID 19 while in their care.
I have read and understand the tuition structure as listed on the Hybrid Support Program Rates Guide. (Linked Below)
I understand that MLEDP may be required to close for weather or health emergencies. Days missed due to emergency closings will not be refunded or credited.

I give permission for the following release person to pick up my child.  I am confirming that this person will be at the center within 30 minutes of a call from staff and is at least 16 years of age
